Plajer has more than a decade of experience in home

health care, chronic-condition management. Throughout

her nursing career, she has coordinated complex care,

collaborated with physicians and health care

changing healthcare needs. Kim is a Johns Hopkins –

certified Guided Care Nurse and a certified dementia

practitioner.

responders better support you when needed.
